“The Mead Maternal Health & Preventive Care Center at Mercy offers one high standard of care, in one convenient location, to two vulnerable populations: at-risk expectant mothers and high-risk chronically ill patients. Thanks to the foundation’s support, Baltimore’s neediest will receive comprehensive pre-natal services, on-site chronic disease diagnostics and treatment, robust patient navigation and education, and referrals to community resources that will help address socio-economic barriers to wellness. The foundation’s partnership in this work will have tremendous impact on people’s health outcomes.”
